THE FOLLOWING INFORMATION IS TO BE READ AND ACKNOWLEDGED BY EVERY PARTICIPANT BEFORE JOINING CLASS

Your instructor is responsible for the safety of the drills and exercises and their instructions must be followed closely. You must observe the safety of your training partners at all times.

In consideration of my acceptance and my participation in Playing for Life activities I acknowledge that physical exercise can be dangerous and involve risk of serious injury, paralysis and/or death. I hereby acknowledge and agree to assume responsibility for all the risks, whether identified in this information or not, even those risks arising out of negligence of my training partners. My participation is purely voluntary. I assume full responsibility for myself and for any bodily injury.

I understand that any engagement in Playing for Life activities with disabilities, impairments or ailments that could possibly be detrimental or inimical to heart, safety and/or comfort or physical condition should first be approved and recommended by a medical physician.

By participating in Playing for Life activities I guarantee that my insurance covers personal injury and accidents during classes.
